 Office Appointment

Prior to your office appointment, please download, print, and complete the New Patient Paperwork, which includes a New Patient Form, Insurance Form, Acknowledgement and Acceptance of Privacy Notice and Practices (HIPAA), Acknowledgement and Acceptance of OSMI Office Policy Form, and Release of Medical Records Form. Please fax or bring forms with you to your appointment. This will minimize your time spent in our waiting room. Please read our office policies.

Surgery Appointment

The forms include the Request for Pre-Surgery Evaluation. This form must be completed by your doctor and be sent back to us. We must receive it at least three days prior to surgery. The PDF form also includes a list of things to do and NOT do the week prior to your orthopedic procedure or surgery. The last page of instructions is a guide for your post orthopedic surgery (post-surgery) recovery period.

Medical and surgical travel for patients is for patients who wish to journey to major medical centers in search of the leading orthopedic and sports medicine specialists for complex medical or surgical treatment that is unavailable in their own communities.
